Center for Spinal Disorders requires that you bring all necessary items to your appointment to ensure we are properly treating your injury. Please bring all of the items below with you to your initial visit:
• Completed patient forms
• Valid form of identification and insurance card
• MRI or X-Ray films and any records pertaining to your neck/back injury or condition
• List of all medications
• Method of payment – cash, check, Visa, Mastercard, Discover, and American Express
• All phone and fax numbers for your referring doctors and primary care physician
